  • You are responsible for a specific dollar amount for each procedure you and/or your child(ren) receive.
  • You can choose your assigned provider within the contracted network.

Premium rates for the 2017 plan year increased by 10% or more over the 2016 plan year rates for the following plan, due to an increase in dental care costs and other factors.
